HC Disease Surveillance system allows Case Managers and Public Health workers the ability to register large numbers of patients that are suspected Persons Under Investigation (PUI) who may be required to remotely self-monitor and report daily vitals and symptoms. Hundreds or thousands of individuals can be registered, uploaded and managed providing Case Managers dashboards and views for easy tracking, monitoring and reporting of infectious diseases.
The Disease Surveillance notification system sends out daily reporting reminders for the designated incubation period, and allows the PUIs to remotely report their information from a smart phone or any web enabled device. Case managers save time with the ability to manage by exception cases that either fail to report or become symptomatic, needing further attention and referral to a medical facility.
